SmartMed is a digital health service allowing patients with chronic illness to manage their own condition from the comfort of home.
PLEASE NOTE: To use this app you must firstly be assigned to the SmartMed programme by your NHS healthcare provider and be in possession of a SmartMed username and authentication PIN.
SmartMed Antenatal is specifically designed for pregnant women with, or at risk of, gestational diabetes. The SmartMed HomeCare app must be used in conjunction with a SmartMed patient account provided by your healthcare provider.
Initially, patient accounts are available from the NHS London North West Healthcare Trust.
Once the patient has an account and has installed the app, they are able to capture vital signs information including blood pressure and blood glucose levels on a regular basis, at home.
The information can then be viewed by a qualified clinician in the healthcare trust who can advise on any necessary interventions, adjustments to diet or medication.
The overall vision for SmartMed Antenatal is to reduce the amount of complications that can arise during pregnancy due to diabetes. It aims to achieve this without additional burden on the healthcare provider or the patient - allowing more frequent assessments to be undertaken from the comfort of the patient's home.
