Humana CareMatch

The CareMatch app allows care managers affiliated with Senior Bridge/Humana Cares to receive referral alerts right from their Android phone! Using the CareMatch app, affiliate care managers can register for, receive, and respond to referral alerts that are sent out when Humana members discharge from the hospital under the Senior Bridge/Humana Cares Transitions Program. Care managers can define the geographic area they wish to support from within the app, and will only receive referral alerts for members who are within that area. The first registered care manager to respond to a referral alert is granted the referral.
